Through partnerships with acute care and other health care providers, the center strives to improve care transitions, reduce emergency department visits and re-hospitalizations, promoting better health and peace of mind.
From your first day, the interdisciplinary team begins planning with you for your discharge. The team includes registered and licensed nurses, certified nursing assistants, medical director, social workers, dietician, therapy specialists, and chaplain.
